Summary
This study tested a telehealth program called GET2HOME for children with complex chronic conditions leaving the hospital. The goal was to see if the program could reduce unplanned hospital visits and improve quality of life for the child and family. Over 600 children and their families took part after being discharged from a pediatric hospital.
