Summary
This study looks at whether a personalized health plan (PHP) can improve quality of life for frail people aged 70 and older. Researchers will give participants various quality-of-life tests and share the results with their doctor and the patient. The study will follow 120 participants for two years, comparing those who get the PHP to those who receive usual care.
