Summary
This study is testing whether creating a personalized health plan for frail older adults, managed by a team of healthcare professionals, can improve their quality of life. It will involve 120 participants aged 70 and over who are identified as frail but still living independently. The main goal is to see if this coordinated care approach helps people stay healthier, more active, and out of the hospital over two years.
