Summary
This study is testing if providing free boxes of fresh fruits and vegetables, with or without extra health coaching, helps people with chronic heart failure. Researchers want to see if this 'food prescription' approach reduces trips to the emergency room, hospital stays, and death rates over one year. They are enrolling 900 Duke Health patients who have had a recent hospital visit and are at risk of not having enough healthy food.
