Summary

This pilot study is testing a new home monitoring system for people with heart failure. It uses a special scale and an alert guide to help manage fluid buildup, a common problem that leads to hospital visits. The goal is to see if this system can be smoothly added to a patient's regular care plan to help control their condition.
