Summary
This study tests a program called the Family Bridge Program (FBP) designed to help low-income families better navigate hospital care, communicate with doctors, and transition home smoothly. About 728 families of children from diverse backgrounds will either receive the program or standard care plus written resources. The goal is to see if the program improves parents' ability to manage their child's healthcare and reduces stress.
