Summary
This study tests a program called THRIVE that helps Medicaid patients with chronic medical conditions and serious mental illness transition from hospital to home. The program provides a home care nurse within 48 hours and a care team that checks in for 30 days. Researchers will see if this reduces hospital readmissions and emergency visits compared to usual care.
