Summary
This study looks at whether group discussions about advance care planning help older adults (70+) document their healthcare wishes. About 516 participants will either join two group sessions led by a doctor and social worker, or receive mailed materials. The goal is to see if group visits lead to more completed advance care planning forms in medical records.
