Summary
This study is testing a new risk calculator designed to identify people at high risk for developing heart failure before they are actually diagnosed. Researchers will work with 600 adults in Greater Manchester who are aged 50+ and have at least two other health conditions like diabetes or high blood pressure. The goal is to see if this tool can be effectively used in local doctor's offices to find people who might benefit from early, targeted treatments.
