Summary
This study tests a digital tool called DIPAR that sends reminders to adults with type 2 diabetes to get their urine tested for kidney function. The goal is to see if the tool helps more patients get this important yearly check-up. About 200 participants will use the tool through a patient portal. The study focuses on improving monitoring, not curing the disease.
