Summary

This study is testing a digital platform that uses sensors and a mobile app to continuously monitor heart failure patients, including those with heart transplants or ventricular assist devices. The platform collects data like vital signs, activity, and sleep, and shares it with doctors for the intervention group to guide treatment decisions. The goal is to reduce deaths and hospital stays while improving quality of life. About 390 adults aged 18-75 will participate for 18 months.
