Summary

This study will test a new way to manage heart disease risk using a digital, nurse-led remote monitoring program. About 750 adults with heart disease or related conditions will use home devices to track their cholesterol and blood pressure. The goal is to see if this approach helps more people reach their treatment targets and improves their quality of life.
