Smartphones and sensors aim to keep heart patients out of hospital

Summary

This study is testing a new digital platform to help manage patients with serious heart conditions like chronic heart failure, heart transplants, or implanted heart pumps. About 390 participants will use smartphones, smartwatches, and home sensors to send daily health data to their doctors. The goal is to see if this continuous monitoring helps doctors make better treatment decisions, reduces hospital stays, and improves patients' quality of life over 18 months.
