App aims to keep heart failure patients out of hospital

Summary
This study is testing whether a digital care platform can help people with a weakened heart pump (heart failure) stay healthier after leaving the hospital. About 774 patients will either use an app to report daily symptoms and vital signs, which helps doctors adjust medications, or receive their usual follow-up care. The main goal is to see if the digital approach reduces the risk of death or needing to go back to the hospital for heart failure over one year.
