Summary
This study tested whether a digital health program helps people with heart failure recover better after leaving the hospital. The program used smartphone apps for education, self-monitoring, and remote check-ins with a special vest sensor. Researchers wanted to see if this approach improved patients' heart function, walking ability, and quality of life over six months, while reducing trips back to the hospital.
