Summary
This study is testing a new digital communication system called the 'Digital Bridge' to help older adults with multiple health conditions transition more smoothly from the hospital back to their homes. It connects hospital teams with primary care doctors and the patients themselves to improve communication and coordination of care. The goal is to reduce confusion after discharge, improve patients' quality of life, and potentially lower the risk of needing to return to the hospital.
