Summary
This study tests whether a mobile app can help people with diabetes take better care of themselves after they go home from the hospital. The app provides education, reminders, and tracking tools for 90 days. Researchers will compare app users to those who get standard discharge instructions to see if the app improves self-management, blood sugar control, and reduces hospital readmissions.
