Summary
This study tests a two-part program to help low-income Chinese and Latino/a immigrants better manage type 2 diabetes. It connects patients with community health workers for social support and sends educational videos via text message. Researchers will measure if this approach helps lower blood sugar levels and is easy for patients and clinics to use.
