Summary

This study tests a program that helps adults with type 2 diabetes use information from wearable glucose monitors and activity trackers to improve their daily self-care. The goal is to make it easier for people to manage their blood sugar, medications, and healthy habits. About 80 participants will try the program for 4 months to see if it helps them stay on track and avoid complications.
