Summary
This study tested a new system to help doctors refer more patients with diabetes to a proven education program. The program teaches people how to manage their diabetes to control blood sugar and avoid complications. Researchers worked with 22 clinics in Kentucky, using technology to simplify referrals and coaches to help clinics improve their processes.
