Summary
This study is testing a new program called DECLARE that aims to better support family members caring for loved ones with dementia at home. The program involves caregivers completing a short assessment about their needs and experiences, which is then shared with their home health care team to help connect them with a social worker. Researchers want to see if this approach makes caregivers feel more confident in their role and increases their access to professional support compared to usual care.
