Summary

This study looks at whether giving extra support to care teams at community health centers helps them better screen patients for non-medical needs—like food, housing, or transportation—and connect them to community services. About 20 health centers will take part. The goal is to improve how clinics use their electronic health records to address these social factors that affect health.
