Summary
This study tests a program called De-HF that helps heart failure patients and their family caregivers work as a team to manage the condition at home. The program includes face-to-face and online support to improve self-care, quality of life, and reduce hospital readmissions. About 232 patient-caregiver pairs will take part after the patient leaves the hospital.
