Summary
This study is testing a simple paper tool to help people with heart failure track their weight daily. The goal is to see if this easy-to-use tool, taught by nurses, helps patients manage their condition better and feel better in their daily lives. Researchers will enroll 42 older adults to see if the methods work well before planning a larger future study.
