Summary
This study is testing a new nurse-led care program called COORDINATE, designed to help older adults who have two or more chronic health conditions and are leaving a special hospital unit. The program provides personalized support, helps patients set health goals, and tracks symptoms through phone or video calls over three months. Researchers want to see if this approach improves quality of life and reduces emergency room visits and hospital readmissions compared to standard care with extra support.
