Summary
This study is testing a community support program called CommunityRx-Kidney Health (CRx-K) for people with chronic kidney disease (CKD) living in rural North Carolina. The program provides patients with education, a personalized list of local health and social resources (like food, housing, and transportation help), and a clinic navigator for 12 months. Researchers want to see if this extra support helps patients manage their condition better and reduces their need for emergency room visits and hospital stays compared to usual care.
