Summary

This study aims to help control high blood pressure in Chicago's South Side, where rates are high and resources are low. About 2,000 adults from local churches and clinics will take part in a program led by community health workers. The program uses proven methods like simplified treatment plans and regular blood pressure checks, adapted to fit the community. The goal is to see if this approach can improve blood pressure control and be sustained long-term.
