Summary
This study is testing whether adding a community health worker to a kidney care team helps more patients with advanced kidney disease choose and start dialysis at home. Researchers will compare two groups: one gets standard team care plus a community health worker, and the other gets standard care alone. They want to see if the extra support helps patients manage social needs like housing or transportation and makes them more comfortable choosing home treatment.
