Summary
This study tests whether training and supporting small groups of residents to improve their local environment—like adding walking trails or healthier food options—can help prevent heart disease. Over 2,400 adults in rural Texas and New York communities will participate, either by leading projects or by having their health tracked. The main goal is to see if these community changes lead to better heart health scores, weight, diet, and physical activity for everyone involved.
