Summary
This study is testing a community support program to help adults in Pokhara, Nepal, manage chronic health conditions like high blood pressure, type 2 diabetes, and smoking. It will see if regular home visits from trained local health volunteers, combined with supportive text messages, leads to better health outcomes compared to usual care. The goal is to find practical ways to help people control these conditions in their daily lives.
