Summary

This study tests whether a proven blood pressure control program can work when delivered through churches in Chicago's underserved South Side. The program uses community health workers and ministry facilitators to help adults manage their high blood pressure through monitoring, simplified treatment, and support. Researchers aim to see if this community-centered approach can successfully lower blood pressure and reduce heart disease risk where traditional healthcare has struggled.
