Summary
This study is testing if a special coaching program can help homeless veterans stay healthier and reduce their frequent need for emergency room visits and hospital stays. It involves 177 veterans who will work with trained peer coaches using a 'Whole Health' approach that focuses on their personal goals and values. The goal is to see if this support leads to better health, more stable housing, and fewer costly hospital visits.
