Summary
This small study is practicing how to use a new discussion checklist called 'CKD Topics' with people who have advanced kidney disease, especially those living in rural areas. The goal is to see if the steps for finding participants, using the checklist, and asking survey questions work properly before starting a larger trial. The checklist is meant to help patients and doctors plan what to talk about during medical visits.
