New approach aims to improve care coordination for chronic pain patients

Summary
This study tests a new way of organizing care for people with long-term pain who are seeing a physical therapist. The goal is to see if a personalized care model, called GAP-421, can improve how well different healthcare providers work together for each patient. About 66 adults with chronic non-cancer pain will take part across three health centers in Madrid, Spain. The study focuses on whether this approach is practical and whether patients feel their care is better coordinated.
