Summary

This study tests a special way for nurses to work with people who have long-term health conditions like diabetes or heart disease. The goal is to help patients become more involved in their own care and make daily decisions about their health. About 420 adults will take part, and researchers will measure how well patients and nurses work together using a simple questionnaire.
