Community health workers may boost home dialysis use in High-Risk patients

Summary

This study tests whether adding a community health worker to a kidney care team helps more patients choose or start home dialysis. About 106 adults with advanced kidney disease will be split into two groups: one gets standard team care, the other also gets a health worker to address social needs and boost engagement. Researchers will track home dialysis use and patient activation over one year.
