Summary

This study is testing if sending people with diabetes home from the hospital with a continuous glucose monitor (CGM) helps them manage their blood sugar better than the traditional finger-stick method. The goal is to see if better daily glucose control leads to fewer trips back to the emergency room or hospital. Researchers will follow 120 veterans for 3 months after discharge to compare the two approaches.
