Summary
This study tests whether giving people with diabetes a continuous glucose monitor (CGM) when they leave the hospital helps them control their blood sugar better than traditional finger-stick testing. 120 participants will be randomly assigned to use either a CGM or finger sticks for 3 months after discharge. The goal is to see if the CGM leads to fewer hospital readmissions and better overall glucose management.
