Summary

This study compares two ways to manage diabetes after hospital discharge: a continuous glucose monitor (CGM) that checks sugar levels automatically versus traditional fingerstick checks. About 64 adults with type 2 or steroid-induced diabetes will wear a sensor for 28 days and have telehealth visits. The goal is to see if CGM improves blood sugar control and reduces dangerous highs and lows.
