Summary
This study tested a program called CAPABLE, which sends a team of an occupational therapist, a nurse, and a handyman to help older adults at home after they leave the hospital. The goal was to see if this support could improve their ability to perform daily tasks like walking, bathing, and dressing, helping them stay independent and avoid moving to a care facility. The program was compared against usual care services in a group of 268 older adults.
