Summary
This study is testing a new patient-centered care model called FRAME, designed to help cancer survivors manage long-term side effects after their main treatment ends. It involves patients filling out a questionnaire about their needs before a clinic visit, then having a focused conversation with their doctor to create a personalized care plan. The research will see if this approach helps doctors better identify patient needs and connect survivors to the right support services.
