Summary
This study follows 50 young adults with chronic inflammatory rheumatic diseases to see which transition program works best when they switch from pediatric to adult care. Some patients received a special education workshop plus a joint consultation, while others had only the consultation. The goal is to measure how ready they are to manage their own health after the move.
