Summary

This study tested if a special, intensive follow-up program could help people better manage their health after a stroke or heart attack. It compared care from a team of nurses, doctors, and pharmacists working together to the usual follow-up care patients receive. The main goal was to see if this team approach was more effective and cost-efficient at preventing repeat events and hospital visits.
