Summary
This study looks at whether a coordinated care plan between general practitioners and physical therapists can help reduce disability in people with chronic or at-risk low back pain. 123 adults aged 18-65 with certain risk factors are participating. The main goal is to measure changes in disability after 3 months using a standard questionnaire.
