Summary
This study tested a new support program for people with coronary artery disease (CAD) who are at high risk of being readmitted to the hospital. The program combined a smartphone app for tracking health with regular check-ins from a nurse to help patients manage medications and lifestyle. Researchers compared this program to standard care to see if it could reduce hospital readmissions or death over 12 months.
