Summary

This study tests whether an AI-powered mobile app, called SMART-CHD, can improve outcomes for adults aged 18-45 with premature coronary artery disease after they leave the hospital. Participants use the app and wearable sensors to track their health, receive reminders, and get personalized advice. Researchers will compare this approach to standard care to see if it reduces deaths, heart attacks, strokes, and hospital readmissions over 12 months.
