Summary
This study is testing a new AI tool that predicts which patients with chronic diseases are likely to have high medical costs. The goal is to see if offering these patients extra support from a nurse care manager can help them stay healthier and avoid hospital visits. Researchers will compare 5,000 patients who get the extra support to a similar group who are placed on a waitlist.
