Summary

This study tests a new digital tool called iMatter2 that uses artificial intelligence to help people with type 2 diabetes track their symptoms and improve blood sugar control. About 353 adults with uncontrolled diabetes will use the tool or receive usual care for 12 months. The goal is to see if the tool leads to better blood sugar levels and helps doctors make more informed decisions.
