Summary

This study is testing a new strategy to help people living with HIV stay healthier and avoid emergency room visits. A computer algorithm identifies patients at highest risk of needing emergency care in the next two weeks. Care managers then use this list to prioritize outreach and support for those patients, compared to their usual methods. The goal is to see if this data-driven approach helps get the right support to people at the right time.
