Summary
This study is testing a new strategy for managing care for people living with HIV. It uses a computer algorithm to predict which patients are most at risk of needing emergency care in the next two weeks. Care managers then focus their outreach on these high-priority patients to try to prevent hospital visits and improve overall health outcomes.
